Patients are invited to participate in a survey designed by East & North Herts Clinical Commissioning Group. The survey is to obtain your views on the practice and locally commissioned services. We would be grateful if you could spare the time to complete this. If you are unable to complete electronically, paper copies of the survey are available from reception.
